Provides assistance with ADLs, medication assistance, pureeing foods, feeding, mobility and transfers including hoyer lift.
Provider lives on site, registered nurse on duty, awake staff, activities coordinator.
Accepts hospice, home health care.
Podiatry, hairdresser, primary care provider from outside providers.
Outdoor pathway, outdoor garden, private visiting area, private and shared rooms, cable/telephone ready.
Small pets allowed. No smoking policy.
Base Rate $4,500 per month.
Accepts Private Pay and Medicaid after 4 years private pay.
